A PRC ID replacement is for a Professional Identification Card (PIC) that has been lost or damaged. It is not the same as normal PIC renewal and it is not the same as replacing the paper Certificate of Registration (COR). PRC currently routes the replacement through LERIS/PRC Online Services plus an appointment.

Lost PIC vs. damaged PIC

Situation Core evidence
PIC was lost System-generated Action Sheet plus Affidavit of Loss
PIC is damaged System-generated Action Sheet plus the damaged PIC
Another person will submit/claim for you Representative documents depend on whether the representative is a registered professional

Step 1: Log in to PRC LERIS

Use the official PRC Online Services/LERIS account. Select the replacement/duplicate PIC transaction, choose an appointment date and place, and use the payment option/channel shown by PRC. Save or screenshot the system-generated Action Sheet because PRC lists it as a basic requirement.

Step 2: Prepare the lost-or-damaged proof

For a lost card, prepare the Affidavit of Loss. For a damaged card, bring the damaged PIC. Do not treat a normal renewal as a replacement transaction merely because the card is near expiry; renewal and replacement are different PRC services.

Step 3: Pay the official fee

PRC’s current FAQ lists the application fee for a duplicate PIC at ₱250. Follow the amount and payment instruction generated by the official system in case a later issuance changes the fee or adds transaction-channel charges.

Step 4: Attend the appointment

Present the Action Sheet and replacement requirements at the designated PRC window. Make sure the professional name, profession and license number match the PRC record. If the professional data itself is wrong, a separate petition for updating the professional record may be needed before or alongside the replacement process.

If an authorized representative will handle the transaction

Representative Current PRC requirement
Non-registered professional Valid government-issued ID plus original Special Power of Attorney executed by the applicant
Registered professional Valid PIC/e-PIC plus original authorization letter signed by the applicant

Release

PRC’s FAQ says a duplicate PIC is released within the day. Still allow time for appointment queues, record checks, payment confirmation, or data discrepancies. Sign the releasing log when the replacement is issued.

PRC ID replacement vs. COR replacement

The PIC is the professional identification card. The COR is the Certificate of Registration. Losing one does not automatically mean you need to replace both. Use the transaction that matches the document actually lost or damaged.

CitizenGuide practical notes

Useful details before you continue

PIC replacement and PIC renewal are separate PRC transactions.

PIC replacement is also different from replacing the Certificate of Registration (COR).

PRC currently states same-day release for a duplicate PIC, subject to complete requirements and appointment processing.

Troubleshooting

If something goes wrong

Start with the closest issue below before creating another application or account.

My name or professional data is wrong in LERIS

Check whether a petition for updating professional data/record is required before expecting the replacement PIC to reflect the corrected information.

I lost the PIC and also need to renew it

Ask PRC which sequence applies to your current record; replacement and renewal are separate services and should not be mixed without confirmation.

A representative will file for me

Use the PRC representative rule that matches whether the representative is a registered professional or a non-registered professional.
