A PRC Certificate of Registration replacement is for the paper/registration certificate (COR), not the Professional Identification Card (PIC). If you lost or damaged only your PRC ID card, use the PIC replacement service instead.

Which PRC document do you need to replace?

Document Use this transaction?
Certificate of Registration (COR) Yes—use the COR replacement transaction
Professional Identification Card (PIC/e-PIC) No—use the PIC replacement service
COR with professional data that has changed You may also need the approved petition/update record supporting the corrected professional data

Current basic requirements

  • Screenshot of the system-generated Action Sheet.
  • Original Affidavit of Loss for a lost COR, or the damaged COR when applicable.
  • Valid PIC/e-PIC.
  • Copy of an approved petition for updating professional data/record when that applies.
  • One Documentary Stamp Tax per copy.

Step 1: Use LERIS to secure the appointment

Log in through PRC Online Services, choose the COR replacement transaction, select an appointment date/place and follow the payment channel. Keep the system-generated Action Sheet for the appointment.

Step 2: Pay the current COR replacement fee

PRC currently lists the COR replacement fee at ₱300, plus one Documentary Stamp Tax per copy. Use the amount produced by the official system if the fee schedule is later updated.

Step 3: Present the correct lost/damaged evidence

For a lost certificate, bring the original notarized Affidavit of Loss required by PRC. For a damaged certificate, bring the damaged COR. Bring the valid PIC/e-PIC because PRC uses it to verify the professional registration record.

Step 4: If the professional data changed, bring the approved update record

A COR replacement is not a shortcut for correcting professional data. If your name or other registered professional information has already been changed through PRC’s petition process, bring the approved petition/update document required for the replacement to reflect that record.

Authorized representative rules

Representative type Requirement
Non-registered professional Valid government ID plus original SPA executed by the applicant
Registered professional Valid PIC/e-PIC plus original authorization letter signed by the applicant

Release

PRC’s current FAQ says the replacement COR is released on the appointment date. Sign the releasing log after receiving the duplicate and inspect the name, profession, registration number and other printed information before leaving.

CitizenGuide practical notes

Useful details before you continue

COR replacement and PIC replacement are separate PRC services.

A data correction must be handled through the appropriate PRC update/petition process; a replacement alone does not correct the underlying record.

PRC currently states that COR replacement is released on the appointment date when requirements are complete.

Troubleshooting

If something goes wrong

Start with the closest issue below before creating another application or account.

I lost both my PIC and COR

Treat them as separate PRC replacement transactions and confirm the correct appointment/payment sequence with PRC.

My COR has my old name

Check whether the professional-data update/change-of-status petition must be approved first, then bring the approved update record for COR replacement.

I cannot personally attend

Use the PRC representative requirement that matches the representative’s professional-registration status.
